If you’re a NEW PATIENT, you MUST e-mail the office first at wheatonhealthcarellc@gmail.com to receive your log-in information in order to initially set up an appointment. Please include this basic information:
- First and last name
- Date of birth
- Phone number
- Email address and/or additional contact information
If you’re an EXISTING PATIENT, please visit our online scheduling platform by clicking the button below to register for an account and/or log in to make an appointment.
NOTE: PLEASE CHECK YOUR SPAM FOLDER. Emails from our online scheduling platform should be almost instant. If you do not see it in your inbox, please double check your spam/junk email folder.
